Why does LOA charge for the completion of forms?
It is the policy of LOA to charge a minimal fee for the completion of forms. This fee is necessary to assist with the payment of time spent by the physician and staff to complete your form(s).

Why is it necessary for me to complete additional paperwork when the physician treated me at the emergency room?
When a LOA physician sees you at the emergency room, the paperwork that you complete is for the hospital. When you come to the physicians office for a follow up, you need to complete similar paperwork for the medical office. LOA must have a record of your medical history, your demographic information, and your insurance information so that we can submit charges to your insurance company for your medical treatment.

How do I get a prescription filled?
You need to call the office between the hours of 8:30 am and 3:00 pm, Monday through Friday. When you call please be prepared to provide the name of your physician, the medication you need refilled, the Rx number, the pharmacy where you want the medication refilled, your date of birth, and the phone number where you can be reached. You need to call when you have 2 to 3 day supply left so that your physician has time to reorder your medication.

Does LOA participate with my insurance?
You need to contact our office at 717-272-7971 for a listing of participating insurance companies with LOA.

What should I do if my employer changes insurance companies?
If you are currently receiving treatment and have a pending appointment, please bring your new insurance information/card with you and provide this information to the receptionist. You can call the office at 717-272-7971 to update your insurance information.
